Home Amenities

Southlake Residential Care Village has been completely rebuilt to provide a warm, comfortable, state-of-the-art environment for all of our 192 long-term care and 32 Interim Unit Residents. Our Home is located on the Southlake Regional Health Centre campus, but is independent of the hospital.

Basic Care Programs and Services

The Long-Term Care Home Act, 2007 states that every licensee of a long-term care Home shall ensure basic care and services are available at no additional cost.

Our Programs & Services

Resident’s Attending Physician

As part of our primary care, Residents are assigned to a specific physician.  Appointments for specialists will be arranged through the Home.

A Resident or the Resident’s substitute decision-maker may retain a physician or registered nurse in the extended class to perform the following services:

  • a physical examination of the Resident upon admission and an annual physical examination thereafter, and to produce a written report of the findings of the examination;
  • attend regularly at the Home to provide services, including assessments; and
  • participate in the provision of after-hours coverage and on-call coverage.

If a Resident chooses to retain their own primary care provider (MD or RN Extended Class usually called Nurse Practitioners) these practitioners must meet the Home’s standards.

Diet & Nutrition

Good nutrition and attractive tasty meals are an essential part of our service.  The Home has a three-week seasonal menu plan.  Nutritional goals are achieved through our Director of Food Services and staff, with the recommendations from the Consulting Dietitian, the Resident’s Food Committee and you.  Three meals a day plus three nutritious snacks are provided for the Resident.  Alternative selections are available and Residents are made aware of the choices at each meal.  Special and therapeutic diets are prepared in accordance with doctor’s orders and assessment by the dietitian is provided if the need arises.  Feeding devices are also available for Residents, as the need is indicated (e.g. rimmed plates, special cups, built up utensils). For the most part, meal service is provided in the dining rooms.  However, if a Resident is ill, meals may be provided in his/her room.  Families are encouraged to purchase meal tickets and dine with their loved ones. Meal tickets can be purchased at reception. Special luncheons are scheduled seasonally for families at Christmas, Thanksgiving, etc. in the auditorium for a fee.

Life Enrichment Program

The Life Enrichment Department strives to provide high quality programs to meet the social, emotional, physical, cognitive and spiritual needs of all Residents.

Programs are offered seven days a week, at times convenient to Residents.  Programs come in many different forms, from individual visiting to large group programs, both within the Home and outside in the community.

Some examples of our Life Enrichment programs include: beauty hour, picnics, barbecues, music, exercise groups, sensory stimulation, games night, karaoke, baking, current events, social tea, knitting groups, gardening and planting pots, movies, reminiscing and trivia contests.  A Pet Therapy Program is held weekly. Life Enrichment programs are also held in the community.  Some of the events and activities have additional costs and the Resident may be requested to help offset the cost involved (for example, a wheelchair bus trip). All Residents are encouraged to participate in programs of their choosing.  Family members are always welcome.

A kitchen and dining area is available on the 1st floor for family get-togethers, in addition you can book our Celebration Dining room through reception to have a meal together as a family (plates and utensils are provided).  Residents and families are welcome to enjoy our gorgeous courtyard and balconies in the warmer months.

Physiotherapy

Residents are assessed by the Home’s physiotherapist and are provided physiotherapy as required.

Pastoral Services

Pastoral Services are coordinated and monitored through the Life Enrichment Department.  We strive to meet the spiritual needs of all Residents.  Participation in spiritual programs is at all times voluntary.  We hold nondenominational church services in our chapel on Wednesdays, and Catholic church services on Thursdays.  Bible study is held every Sunday.  Every 3 months a memorial service is held for Residents and family to attend.  For more details or special requests please contact the Life Enrichment Manager.

Residential Care Conferences

At the time of admission, a multidisciplinary assessment is completed for each Resident, and a plan of care is formulated, with input from the Resident and family.  Within the first 6 weeks after admission, a conference is held for each Resident.  A plan of care will be reassessed at the conference, including an Advance Care Planning discussion.  This conference is then held at least annually and offers an opportunity for all parties to communicate and maintain a consistent approach to care and quality of life for the Resident.  This conference is an excellent opportunity to share information in an informal setting, at a scheduled time, with all departments represented.

Additional Care Services

Podiatry/Foot Care Nurses

A specialized foot care nurse is provided on-site to all our Residents.  There is an additional fee for these visits.

Dental Services

Dental Services are provided through Direct Dentistry at Southlake Village.

Dentist:  Arrangements may be made for dental visits through the charge nurse when necessary.  We recommend an annual examination by a dentist for all Residents.

Denturist:  Arrangements to have dentures made, relined, adjusted may be made by contacting the charge nurse or by contacting a denturist of your choice.  The denture therapist will provide services within the long term care Home.  The cost of these services is the responsibility of the Resident.

Pharmacy Services

Medical Pharmacy provides pharmacy services and medications for Southlake Village.  Medications/Treatments are prescribed by the doctor only and are administered by the nursing staff.  No medication of any kind may be brought to the Resident or left in the Resident’s room.  Not all medications/treatments are covered by the Ontario Drug Benefit Program (OBD) or the Government Pharmacy.  There may be a charge for drugs not covered by the government, however the P.O.A will be notified of such charges before the medication is ordered.

Hair Styling

Beautician/Barber services are provided by appointment as requested by either the Resident or family member.  The cost of these services is the responsibility of the Resident.

Mending & Sewing Services

Mending and sewing services can be available for a small additional fee.  Services will be provided at the request made by Resident/family members.
