Our Home is located on the Southlake Regional Health Centre campus, but is independent of the hospital.[\/vc_column_text][\/basic_container][\/vc_column][\/vc_row][vc_row][vc_column][basic_container][alternating_content_container]\t<div class=\"alternating-content-item-wrapper transparent\">\n\t\t\t\t\t<div class=\"alternating-content-item-image start-bg-center\" style=\"background-image: url('https:\/\/stage.tbkdev.com\/dev\/southlakecarevillage.com\/src\/app\/uploads\/2019\/09\/DSC04987-1024x768.jpg');\"><\/div>\n\t\t\t\t<div class=\"alternating-content-item-content-container\">\n\t\t\t<div class=\"alternating-content-item-inner\">\n\t\t\t\t\t\t\t\t\t<h2 class=\"alternating-content-item-heading\">\n\t\t\t\t\t\tAmenities include:\t\t\t\t\t<\/h2>\n\t\t\t\t\t\t\t\t\t\t\t\t\t<div class=\"alternating-content-item-content\">\n\t\t\t\t\t\t<ul>\n<li>Automated climate control<\/li>\n<li>Attractively designed and decorated dining rooms<\/li>\n<li>Outdoor balconies on every level<\/li>\n<li>A secure, landscaped courtyard<\/li>\n<li>Chapel<\/li>\n<li>Beauty salon\/barbershop<\/li>\n<li>Family dining room for special family events<\/li>\n<li>Comfortable lounges and activity rooms<\/li>\n<li>Auditorium for large parties and activities<\/li>\n<\/ul>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t<\/div>\n[\/alternating_content_container][\/basic_container][\/vc_column][\/vc_row][vc_row][vc_column][basic_container container_size=&#8221;container container-narrow&#8221; text_align=&#8221;text-center&#8221;][vc_column_text]<\/p>\n<h4><strong>Basic Care Programs and Services<\/strong><\/h4>\n<p>The\u00a0<em>Long-Term Care Home Act, 2007<\/em> states that every licensee of a long-term care Home shall ensure basic <a href=\"https:\/\/stage.tbkdev.com\/dev\/southlakecarevillage.com\/src\/app\/uploads\/2019\/09\/CARE-AND-SERVICE-EXPECTATIONS.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">care and services<\/a> are available at no additional cost.[\/vc_column_text][\/basic_container][\/vc_column][\/vc_row][vc_row][vc_column][basic_container container_size=&#8221;container container-narrow&#8221;][vc_column_text]<\/p>\n<h2>Our Programs &amp; Services<\/h2>\n<p>[\/vc_column_text][vc_toggle title=&#8221;Resident\u2019s Attending Physician&#8221;]As part of our primary care, Residents are assigned to a specific physician.\u00a0 Appointments for specialists will be arranged through the Home.<\/p>\n<p>A Resident or the Resident\u2019s substitute decision-maker may retain a physician or registered nurse in the extended class to perform the following services:<\/p>\n<ul>\n<li>a physical examination of the Resident upon admission and an annual physical examination thereafter, and to produce a written report of the findings of the examination;<\/li>\n<li>attend regularly at the Home to provide services, including assessments; and<\/li>\n<li>participate in the provision of after-hours coverage and on-call coverage.<\/li>\n<\/ul>\n<p>If a Resident chooses to retain their own primary care provider (MD or RN Extended Class usually called Nurse Practitioners) these practitioners must meet the Home\u2019s standards.[\/vc_toggle][vc_toggle title=&#8221;Diet &amp; Nutrition&#8221;]Good nutrition and attractive tasty meals are an essential part of our service.\u00a0 The Home has a three-week seasonal menu plan.\u00a0 Nutritional goals are achieved through our Director of Food Services and staff, with the recommendations from the Consulting Dietitian, the Resident\u2019s Food Committee and you.\u00a0 Three meals a day plus three nutritious snacks are provided for the Resident.\u00a0 Alternative selections are available and Residents are made aware of the choices at each meal.\u00a0 Special and therapeutic diets are prepared in accordance with doctor\u2019s orders and assessment by the dietitian is provided if the need arises.\u00a0 Feeding devices are also available for Residents, as the need is indicated (e.g. rimmed plates, special cups, built up utensils). For the most part, meal service is provided in the dining rooms.\u00a0 However, if a Resident is ill, meals may be provided in his\/her room.\u00a0 Families are encouraged to purchase meal tickets and dine with their loved ones. Meal tickets can be purchased at reception. Special luncheons are scheduled seasonally for families at Christmas, Thanksgiving, etc. in the auditorium for a fee.[\/vc_toggle][vc_toggle title=&#8221;Life Enrichment Program&#8221;]The Life Enrichment Department strives to provide high quality programs to meet the social, emotional, physical, cognitive and spiritual needs of all Residents.<\/p>\n<p>Programs are offered seven days a week, at times convenient to Residents.\u00a0 Programs come in many different forms, from individual visiting to large group programs, both within the Home and outside in the community.<\/p>\n<p>Some examples of our Life Enrichment programs include: beauty hour, picnics, barbecues, music, exercise groups, sensory stimulation, games night, karaoke, baking, current events, social tea, knitting groups, gardening and planting pots, movies, reminiscing and trivia contests.\u00a0 A Pet Therapy Program is held weekly. Life Enrichment programs are also held in the community.\u00a0 Some of the events and activities have additional costs and the Resident may be requested to help offset the cost involved (for example, a wheelchair bus trip). All Residents are encouraged to participate in programs of their choosing.\u00a0 Family members are always welcome.<\/p>\n<p>A kitchen and dining area is available on the 1<sup>st<\/sup> floor for family get-togethers, in addition you can book our Celebration Dining room through reception to have a meal together as a family (plates and utensils are provided).\u00a0 Residents and families are welcome to enjoy our gorgeous courtyard and balconies in the warmer months.[\/vc_toggle][vc_toggle title=&#8221;Physiotherapy&#8221;]Residents are assessed by the Home\u2019s physiotherapist and are provided physiotherapy as required.[\/vc_toggle][vc_toggle title=&#8221;Pastoral Services&#8221;]Pastoral Services are coordinated and monitored through the Life Enrichment Department.\u00a0 We strive to meet the spiritual needs of all Residents.\u00a0 Participation in spiritual programs is at all times voluntary.\u00a0 We hold nondenominational church services in our chapel on Wednesdays, and Catholic church services on Thursdays.\u00a0 Bible study is held every Sunday.\u00a0 Every 3 months a memorial service is held for Residents and family to attend.\u00a0 For more details or special requests please contact the Life Enrichment Manager.[\/vc_toggle][vc_toggle title=&#8221;Residential Care Conferences&#8221;]At the time of admission, a multidisciplinary assessment is completed for each Resident, and a plan of care is formulated, with input from the Resident and family.\u00a0 Within the first 6 weeks after admission, a conference is held for each Resident.\u00a0 A plan of care will be reassessed at the conference, including an Advance Care Planning discussion.\u00a0 This conference is then held at least annually and offers an opportunity for all parties to communicate and maintain a consistent approach to care and quality of life for the Resident.\u00a0 This conference is an excellent opportunity to share information in an informal setting, at a scheduled time, with all departments represented.[\/vc_toggle][\/basic_container][\/vc_column][\/vc_row][vc_row][vc_column]\t<div class=\"simple-image-bar count-5\">\n\t\t<ul class=\"simple-image-bar-list\">\n\t\t\t\t\t\t\t<li>\n\t\t\t\t\t\t\t\t\t\t<img decoding=\"async\" container-narrow&#8221;][vc_column_text]<\/p>\n<h2>Additional Care Services<\/h2>\n<p>[\/vc_column_text][vc_toggle title=&#8221;Podiatry\/Foot Care Nurses&#8221;]A specialized foot care nurse is provided on-site to all our Residents.\u00a0 There is an additional fee for these visits.[\/vc_toggle][vc_toggle title=&#8221;Dental Services&#8221;]Dental Services are provided through Direct Dentistry at Southlake Village.<\/p>\n<p><u>Dentist:<\/u>\u00a0 Arrangements may be made for dental visits through the charge nurse when necessary.\u00a0 We recommend an annual examination by a dentist for all Residents.<\/p>\n<p><u>Denturist:<\/u>\u00a0 Arrangements to have dentures made, relined, adjusted may be made by contacting the charge nurse or by contacting a denturist of your choice.\u00a0 The denture therapist will provide services within the long term care Home.\u00a0 The cost of these services is the responsibility of the Resident.[\/vc_toggle][vc_toggle title=&#8221;Pharmacy Services&#8221;]Medical Pharmacy provides pharmacy services and medications for Southlake Village.\u00a0 Medications\/Treatments are prescribed by the doctor only and are administered by the nursing staff.\u00a0 No medication of any kind may be brought to the Resident or left in the Resident\u2019s room.\u00a0 Not all medications\/treatments are covered by the Ontario Drug Benefit Program (OBD) or the Government Pharmacy.\u00a0 There may be a charge for drugs not covered by the government, however the P.O.A will be notified of such charges before the medication is ordered.[\/vc_toggle][vc_toggle title=&#8221;Hair Styling&#8221;]Beautician\/Barber services are provided by appointment as requested by either the Resident or family member.\u00a0 The cost of these services is the responsibility of the Resident.[\/vc_toggle][vc_toggle title=&#8221;Mending &amp; Sewing Services&#8221;]Mending and sewing services can be available for a small additional fee.\u00a0 Services will be provided at the request made by Resident\/family members.[\/vc_toggle][\/basic_container][\/vc_column][\/vc_row][vc_row][vc_column][basic_container
